Modesto, California Aug 12, 2026 (Issuewire.com) - Crystal Lake-Akinnuoye, MSN, FNP-C, RN, is a dedicated Family Nurse Practitioner serving the Modesto–Merced region of California with nearly two decades of progressive healthcare experience. Through her commitment to evidence-based practice, patient education, and whole-person care, she has built a career focused on improving health outcomes while addressing the physical, mental, and behavioral health needs of diverse patient populations.
Crystal currently practices at Riverbank Community Health Center under the Oak Valley Hospital District, where she provides comprehensive primary care for patients across the lifespan. Her clinical expertise includes chronic disease management, geriatric care, mental health, and substance use disorder treatment. She is especially passionate about integrating behavioral health awareness into routine primary care, recognizing the important connection between mental and physical wellness in achieving lasting health outcomes.
Further expanding her commitment to community health, Crystal is in the final stages of vendor approval as a provider with Valley Mountain Regional Center (VMRC). Once approved, she will provide specialized healthcare services for individuals with developmental and intellectual disabilities, helping improve access to comprehensive, community-based care for one of California’s most underserved populations.
Building on this mission, Crystal is also preparing to launch Empowered Living Nursing Services, a new healthcare practice that will provide specialized nursing care and support for individuals with developmental and intellectual disabilities under her Registered Nurse (RN) license. The practice will focus on delivering compassionate, person-centered services that promote independence, enhance quality of life, and provide families with trusted, individualized care. Through Empowered Living Nursing Services, Crystal aims to expand access to high-quality community-based care while addressing the unique healthcare needs of a population that has historically faced significant barriers to services.
Crystal’s journey in healthcare reflects determination, lifelong learning, and a passion for serving others. She began her career in 2002 as a ward secretary before steadily advancing through numerous healthcare roles, including certified medical assistant, licensed practical nurse, registered nurse, and ultimately Family Nurse Practitioner after earning her Master of Science in Nursing from Chamberlain University in 2018.
In addition to her nursing career, Crystal spent nearly 20 years as a licensed cosmetologist. Working closely with clients from diverse backgrounds strengthened her communication skills and reinforced the importance of listening, empathy, and trust—qualities that continue to shape her patient-centered approach to healthcare today.
Throughout her career, Crystal has gained experience across a broad range of clinical settings, including skilled nursing facilities, rehabilitation centers, home health, hospice, outpatient primary care, and nursing leadership. This diverse background has equipped her with a comprehensive understanding of patient care across multiple stages of life and varying levels of medical complexity.
Committed to professional growth, Crystal holds certifications in Substance Use Disorder Medication-Assisted Treatment (SAMHSA), hepatitis treatment, immunization administration, and developmental and intellectual disability care. She is also completing specialized training in musculoskeletal injections and is currently pursuing certification as a Psychiatric-Mental Health Nurse Practitioner, further strengthening her ability to provide integrated primary and behavioral healthcare.
“What I enjoy most about practicing medicine is meeting new people and taking care of different cultures,” says Crystal. “I prefer to be proactive rather than reactive. Prevention, education, and early intervention are the foundation of better health. At the end of the day, knowing that I’ve done all that I can to help someone else through what they’re going through—that’s what matters to me. I’m very hands-on when it comes to patient education because patients do not get that very often. I focus on detailed teaching and education so my patients truly understand their health and feel empowered to take an active role in their care.”
Looking toward the future, Crystal remains committed to expanding both her clinical impact and leadership within healthcare. Alongside the launch of Empowered Living Nursing Services, she plans to establish a multi-location practice while working toward a future executive leadership role as a healthcare CEO. Guided by a vision of accessible, compassionate, and holistic care, she continues to advocate for underserved communities and advance integrated primary and behavioral healthcare throughout California.
